Here's the final design. There are no formatting changes from the preliminary version. I've attached both the MS Word and PDF versions. Feel free to modify the Word version (or the PDF if you have full Acrobat). I'll repeat the Acrobat printing note:

***  If using Adobe Acrobat, select File -> Print, and then on the Print dialog page, set Page Scaling to None. Otherwise, Acrobat will thoughtfully change the printed size for you, which will mess up the alignment.  ***

I verified the alignment using Avery product number 8873 (Ink Jet 'Clean Edge'). Avery makes many different business card products for ink jet and laser, micro-perf and Clean Edge. They offer different paper finishes and stock colors. Any Avery business card form that contains ten 2" x 3.5" cards per page will work with these files. Here's a list of Avery forms that should work:
